For information or to request your medical records, please contact the Medical Records Department.  For your privacy and security medical records cannot be faxed directly to patients.  Please call to arrange for pick up or mail delivery of your records.

If you would like to receive copies of your medical records or would like us to send copies of your records to another party, please complete the Authorization to Release Medical Information [PDF].  Once completed you may return it to us by fax, by mail or in person.

Location and Hours

The Medical Records Department is located on the lower level of the hospital, enter through Main Entrance West.  Please stop at the lower level security desk for directions.
