Diabetes Self-Management Education

Learning to Manage Your Diabetes

You can learn to manage your own diabetes. Central DuPage Hospital provides a comprehensive diabetes education program so you can learn how to care for yourself. It’s important that you attend each of the sessions and bring with you current information about your blood pressure and laboratory results. If you have questions or for more information, please call 630.208.3345.

Scheduling Your Diabetes Self-Management Appointments

Prior to your first visit, your physician will need to fax information about your blood pressure, laboratory results and provide a referral to the Diabetes Education Office. To schedule your appointments, call 630.208.3345 or call the Diabetes Center at 630.262.6078.

Interpreters are available by appointment, please contact the Diabetes Center for more information.

Your Diabetes Self-Management Training Schedule

To better prepare you to manage your own diabetes, it is recommended that you complete all sessions. With attendance at these sessions, you'll receive your Certificate of Program Completion.

Initial Appointment

Length:  1 hour

Notes:  A1C may be obtained unless faxed by your provider, prior to appointment.**

Topics:

  • Health history
  • What is diabetes?
  • Collaborative goals
  • Blood glucose monitoring and target goals
  • Sick day rules

Session I

When:  Schedule during month 1

Length: 2 hours

Notes: You should attend this class soon after your initial appointment.

Topics:

  • Adjustment to diabetes
  • Sick day management
  • Medications
  • Acute complications
  • Carbohydrate counting and food labels
  • What is A1C?
  • Problem solving

Session II

When:  Schedule during month 1

Length: 2 hours

Notes: You should attend this class soon after Session I.

Topics:

  • Exercise
  • Prevention of chronic complications
  • Cardiovascular health and meal planning
  • Blood glucose targets
  • Setting personal goals

Session III

When:  Schedule during month 1

Length: 2 hours

Notes: Remember to bring your current blood pressure, A1C and lipids to this session.

Topics:

  • Balancing stress
  • Blood glucose target review
  • Foot care
  • Strategies for dining out
  • Personal goals review

Session IV

When:  Schedule during month 2

Length: 2 hour

Notes: Remember to bring your current blood pressure, A1C and lipids to this session.  A1C may be obtained unless faxed by your provider, prior to Session IV.

Topics:

  • Prevention of chronic complications
  • Five good habits for making healthy food choices
  • Blood glucose target review
  • Personal goals
  • Continuing education and resources
  • Program completion certificate

Follow Up Session

When:  Schedule during month 2-3

Length: 1 hour

Notes: Remember to bring your current blood pressure, A1C and lipids to this session.  A1 C may be obtained unless faxed by your provider prior to visit.
