Patient Information and Forms
New patients should arrive at least 15 minutes early so that your initial paperwork can be completed and entered into our systems. Please bring a list of your current medications, including prescriptions and over-the-counter medications, your preferred pharmacy information and immunization records for children, if applicable.
Patient Registration
Please bring your current insurance card to each visit so we can verify your information. Without the insurance card, we will be unable to file a claim with your insurance and you will be responsible for the charges incurred that day. To ensure accuracy of our records and to protect your information, we will verify your name, date of birth and demographic information at each visit.
Please be prepared to pay your office visit co-payment and/or outstanding balances. Additionally, if you do not have insurance coverage for services provided, please be prepared to pay for the current visit. For your convenience, we accept MasterCard, Visa, Discover, cash or check.
Prescription Refills
We request that you ask for prescription refills at the time of your office visit. If you need a prescription refilled outside of your office visit, please contact your pharmacy directly for the refill. They will fax us your prescription refill request and we will work with them to complete the order. Please allow two business days for refills. Emergency refills will be addressed accordingly. On-call physicians will not refill routine or narcotic prescriptions when our office is closed. We reserve the right to refuse to prescribe any medications until a physician can evaluate the patient in their office.
Missed Appointments
We understand that people may have emergencies or may simply forget an appointment. However, if more than one appointment is missed without a 24 hour notice, it may result in a charge to you. If you have missed two or more appointments, without notice, you may be dismissed from our practice.
Patient Forms
At Cadence Physician Group we continually strive to provide excellent care and service to all our patients. That is why forms are now provided online for patients to fill out at their convenience before an appointment or when requesting medical records.
Telephone Calls and Answering Service
There is a physician on-call 24 hours a day, 7 days a week for urgent matters. If you have a question that is not urgent, please call during normal office hours. If you are experiencing chest pain or shortness of breath or other medical emergency, please call 911. Our highest volumes of calls are on Monday mornings and Fridays. To serve you better, please make routine calls on other days or in the afternoons.
