Electronic Medical Record

Our office utilizes a secure electronic medical record (EMR) as your chart. This is new technology that contributes to safer, more accurate and timely care for you, our patient. Please work with us to keep your information accurate. We will order your tests, your prescriptions and document your visits in the EMR.

We hope that you are as excited as we are to move forward in today’s technology.

Release of Medical Records

A signed release is required to obtain medical records. Please allow two to four weeks to copy records. A minimal charge may be assessed for this service. Our fees are based on the Illinois State Laws for Medical Record Copying.

Completion of Forms

As a service to you, we will complete school or work forms at the time of your visit. Some examples of these forms are: sports or school physicals, medical leaves and disability. Should you need additional forms completed, without a physician visit, you may be charged a fee per form completed. Please contact our office for details.
