Insurance
We strive to participate in the same health insurance plans as Cadence Health however, when making an appointment with one of our physicians, we ask that you contact your insurance company to confirm that our physician is currently under contract with your health plan. We do participate in Medicare and Medicaid insurance programs. If you present a valid insurance card at the time of your visit, we will file an insurance claim for you.
Non-covered Services
An “insurance waiver,” also known as an Advanced Beneficiary Notice (ABN), may be required to acknowledge your financial responsibility for non-covered services. You may be asked to sign a waiver for preventative health exams, including school, sports and work physicals. Please verify your preventative care benefits with your insurance carrier. Physicians will not “re-code” visits based on your specific coverage. If you are coming in for a non-covered service, please be prepared to pay for the service in full.
Billing Questions
If you have questions regarding a bill, please call 630.933.1701 between the hours of 8 am – 5 pm, Monday through Friday. Payment plans are available. If you are experiencing financial hardship, please contact our office so we can assist you. If your balance becomes past due, appropriate action will be taken to collect the amount due, including sending your account to a collection agency.
