Comprehensive Management for Diabetes

Living with diabetes presents day-to-day challenges. Left untreated over time, diabetes can cause serious health problems that affect many systems of the body from head to toe. With diligent management, however, individuals may be able to prevent or delay these health problems.

The Diabetes Centre at Central DuPage Hospital offers comprehensive care and support to help adults and children manage diabetes. We work closely with patients, families, primary care physicians, other referring physicians and healthcare professionals to meet the complex needs of individuals with diabetes. Our patient-centered program focuses on clinical, educational and psychosocial needs. Ultimately, our goal is to help patients minimize the devastating complications of diabetes through diligent disease management.

Our Services

The Diabetes Centre provides a full complement of clinical and support services aimed at helping individuals to manage this complex and chronic disease. We work with adults of all ages, children, families and pregnant women (including women with gestational diabetes related to pregnancy).

Our services include:

  • Comprehensive self-management education
  • Pediatric education
  • Conception education (for adults with diabetes who wish to become parents)
  • Diabetes in pregnancy education 
  • Support group for adults
  • Nutritional therapy and counseling
  • Insulin pump therapy
  • Continuous glucose monitoring
  • Coordination with referring physician to assure continuity of care

With a strong focus on educating and empowering patients to manage their symptoms, the Diabetes Centre of Central DuPage Hospital has earned recognition from the American Diabetes Association for quality education in diabetes self management.

The Team

The Diabetes Centre is staffed by a multidisciplinary team of health professionals who are focused exclusively on the needs of people with diabetes. Drawing from their diverse expertise, this team helps patients to address the many physical, social, emotional and lifestyle complexities of living with diabetes, such as measuring and moderating blood glucose levels, managing insulin, exercise, foot care and more.

The diabetes care team centers around one crucial team member: the patient. We view each patient as a very important partner in his or her own care.

Additionally, our professional staff includes:

  • Diabetes clinical nurse specialist
  • Diabetes nurse educators
  • Diabetes dietitians

To refer a patient to The Diabetes Centre at Central DuPage Hospital, fax an order to (630) 752-0952. For more information or to speak with a member of the Diabetes Centre team, call (630) 933-6301.

We pledge to keep you, the referring physician, well informed about your patient’s progress at the Diabetes Centre. When the patient completes the Partners in Care program, he or she will be advised to return to your practice for ongoing clinical care.
